Original Comment At my last visit, I was advised that there was a procedure which the insurance would not cover.<br /> I stated that if I had to pay for it, I did not want it.<br /> So, I signed a form declining the procedure.<br /> My complaint/suggestion is that I should have been given a copy of that form.<br /> The report indicates that I refused the procedure but does NOT indicate that it was based on payment. <br /> Perhaps the form implies that but not having a copy, I do not know the exact verbiage.<br /> Please advise. Thank you.
